CDPAP puts you in charge of your own care. Instead of being assigned a stranger from an agency, you choose who cares for you — often someone you already know and love, like an adult child, sibling, friend, or neighbor — and you decide how care happens, in your own home.
With CDPAP, Medicaid pays for your home care, and a single statewide fiscal intermediary, PPL, handles payroll and all official paperwork. A facilitator is the local partner who guides you through all of it — and that’s us – Concepts of Independence.

- We pioneered CDPAP. Our founders helped create CDPAP in the 1980s, and we’ve tracked every rule change since. When the program shifts, we already know what to do. We have experience, since CDPAP is our only business.

Best of all, once you are deemed Medicaid eligible, it costs you nothing. CDPAP is a Medicaid benefit, and facilitator support is part of it. And if you’re already with another facilitator, you’re free to switch without your care skipping a beat.
